Access to WIC

• Medical/health providers may refer their patients to the WIC Program using a referral form with the client's name, date of birth, height, weight, blood results, lead test results and any other health concerns.
• Families may self-refer by calling the WIC Program directly at (888) 942-2229.

The PHFE WIC Program will provide medical/health providers with WIC information and referral forms to facilitate access to WIC services.
